PLEASE USE A SEPARATE FORM FOR EACH DACHSHUND ENTERED

To enter your dachshund who has gone to the Rainbow Bridge, on this site, please complete the form below. We have spent much time trying to make the form as simple as possible - we realise that you are grieving, and that completing something like this is not easy. We would be grateful if you would read the instructions as you go, thus allowing us to put your memorial up as soon as possible. Thank you for your assistance in this.

Please, please, please note!

This site has been in use since 1995, and just about every name you can think of has been entered, so PLEASE identify your dog with his or her name and your family name - e.g. "Heidi Smith" - or by a nickname - e.g. "Heidi the Sock Hider". This is essential so that you can quickly find your pet in the index and so that we can index him or her correctly!!

Name of dachsie (TWO NAMES PLEASE!!! - Oscar Smith or Oscar the Sock Hider!!!):

We give a butterfly to boy dachsies
and a rose to girls dachsies.

Please tell us which your pupper was.

Sex: MaleFemale

Month and year dachsie went to the Bridge

Age (this may be just years, or years and months):

I would like to let you know when I have put your pet up on the page, and I may need to ask you questions about your entry, so *please include your email below*. It will not be included on the memorial, but I really need a contact email in case I have any queries.

Unless you give me an email in this space, I will, with regret, not be able to include your dachsie on the page.

Email for acknowledgement of your entry:

If you would like to send a picture of your dachsie to be included on the page, attach it to an email and send it after you have sent this form.

It is ESSENTIAL, in this time of digital cameras, that your picture has the NAME OF YOUR DOG - e.g. oscar.jpg - and NOT JUST IMG_123.jpg.

Please be mindful of the fact that I get several dogs' pictures every day and if your picture doesn't bear your dog's name, it could well be misplaced!!

The following response box has plenty of space for your memorial. The space you can see is a standard size, but the actual typing area is very large. Please type in upper and lower case - DO NOT TYPE IN CAPITAL LETTERS!, use punctuation as appropriate, and avoid abbreviations.

This page was originally created as a place to pour out your heart for your dachsie, so don't be shy. No one will laugh at anything you feel you want to write.

Dachshund grieving is understood by others who grieve for their dachsie babies, and those are the people who walk in this garden. No one will read your memorial with anything but love.

You have now entered your dachshund's details. Please check to make sure you have included an email address for questions and returns.
